Guest Speaker Presentation

Outreach Coordinator

Thousands of people who are eligible for full benefits of Medicare and Medi-Cal Long Term Services and Support (LTSS), also referred to as "duals", now have an option to choose a managed care plan to coordinate care. This program is called the California Coordinated Care initiative (CCI) and the coordinated managed care program is referred to as Cal MediConnect. If you are eligible for Medi-Cal LTSS benefits only, not Medicare, you will be required to choose a managed care plan in your county. Many beneficiaries and caregivers may have questions about Cal MediConnect. What is covered under this plan? What are the benefits of joining Cal MediConnect?  We may be able to help providers, caregivers and beneficiaries answer these questions. The decision which option is best is a personal one. Making that decision wisely requires accurate information about the program. Please join us at this outreach and education event. Thanks.
